Job Description

Responsibilities:

Plans,organizes,and conducts a comprehensive program of individual and group nutrition services including provision of nutrition needs of the participant, clinical guidance, nutrition education and counseling to meet normal and therapeutic needs.
Evaluates nutritional status of individual participantsutilizingnutritional guidelines, professionalknowledge,and interviewing skills toprovidequality care.
Develops and implementsnutritional plans,monitorsparticipants progress and recommends/revises changes in nutritional plan for effective participant care.
Documentschangesin the participants condition and details care provided by completing all required documentation and ensuring compliance to standards and policies.
Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as needed.
Providesnutrition counseling to participants and/or caregiversutilizingeducational materials, modifieddiets,and teaching aids to ensure participants understanding ofthe planof care and its implications.
Oversees the work of Geriatric Caregivers in the kitchen by working withFood ServicesManager to recommend solutionsregardingspecific dietary needs.

Qualifications:

NY Registered Dietitian

Education:

Bachelors Degree with a major in food and nutrition orrelated field
